Paperwork and Prices

Whether you will be bringing your cat to All Feline Hospital for the first time, or whether you are a regular visitor, we may ask you to fill out some paperwork when you come in.  If it is your first time with us, we will ask you to fill out some personal contact information.  If it has been more than a year since we last saw you, we may ask you to fill out an information update form.  If you are dropping your cat off with us for surgery or for medical testing or treatment of any kind, we will ask you to fill out a consent form.  If your cat has had vomiting, diarrhea, or inappropriate elimination, we may also ask you to fill out a questionaire to help us diagnose the reason behind it.  If you wish to save some time, you can go to our forms page, print out the appropriate form, and bring it in with you when you come into All Feline Hospital.

If you are price shopping, or wish to know what to expect cost wise when you bring your cat into All Feline, you can click on our prices page for a list of our most common prices, and what is all included with our routine surgeries.  We also have price estimates listed for more advanced surgeries and medical issues that may require several days of hospitalization.  Please know that while all price points are accurate, the estimate ranges are just an estimate, and depending on your specific cat, we may go below or above the price estimate.
